California Music Society (CMS) Enrollment Agreement
This Enrollment Agreement applies to all students enrolled in California Music Society (CMS) programs, rehearsals, performances, workshops, and related activities. CMS provides a structured educational environment focused on musical training, performance experience, and artistic development.
1. General Liability & Assumption of Risk
Participation in CMS programs is voluntary. The participant (or parent/guardian if under 18) acknowledges that music education activities, including rehearsals, performances, workshops, and events, may involve inherent risks, including the possibility of injury or serious harm.
The participant agrees to exercise reasonable care for their own safety and knowingly assumes all risks associated with participation in CMS programs and activities.
The participant releases the California Music Society, including its directors, officers, employees, instructors, volunteers, and affiliates (“CMS Parties”), from any and all claims or liabilities arising from participation.
The participant confirms responsibility for determining fitness to participate and acknowledges this agreement is governed by the laws of the State of California.
2. Code of Conduct
- Treat all participants, faculty, and staff with respect and professionalism
- Avoid harassment, discrimination, or unsafe behavior
- Follow CMS rules, schedules, and expectations
- Respect facilities, instruments, and equipment
- Act responsibly during all program-related activities
CMS may remove any participant whose behavior is unsafe, disruptive, or inconsistent with program standards.
3. Media (Photo, Video, and Audio) Release
Participants grant CMS permission to photograph, film, and record their participation in all program activities. Participants may withdraw permission through a written request.
Media may be used for educational, promotional, fundraising, archival, and organizational purposes across all CMS platforms and materials to be used in perpetuity without compensation provided. CMS may edit, publish, and distribute media with uses including marketing, grants, and outreach.
4. Travel & Event Waiver
Participation in CMS during travel, commute, and off-site programs involves inherent risks. Participants assume full responsibility for conduct and safety.
CMS provides supervision but does not guarantee complete safety. Participants release CMS from liability related to travel and events.
5. Permission to Participate & Consents
- Participant grants CMS permission and consent to the following:
- Consent to participate in CMS programs and performances
- Permission for emergency medical treatment
- Acknowledgment of transportation risks
- Consent for surveys and program evaluation
- Consent for media recording and release
6. Terms and Conditions
- Admission is not guaranteed
- CMS may modify programs at any time
- CMS may remove participants for conduct or policy reasons
- Participation is subject to CMS policies
Acknowledgment
By signing, the participant (or guardian) confirms they understand and agree to all terms of this CMS Enrollment Agreement.
